Health Requirements
Physical Exam is mandated by the Illinois Department of Public Health prior to a child's entrance into school, irrespective of grade, and within one year prior to entering K, 6th, and 9th grade. A current immunization record must also be submitted with the health examination form. A diabetes risk assessment and BMI must also be included with the health examination.
Vision Exam is mandated by the Illinois Department of Public Health prior to kindergarten or first entry into school after kindergarten.
Dental Exam is mandated by the Illinois Department of Public Health for children entering grades K, 2nd, and 6th. This dental exam must have taken place within 18 months prior to May 15th of the school year. A licensed dentist must perform the examination and sign the state report form.
Vision & Hearing Screenings
Hearing Screening is mandated by the Illinois Department of public health and performed annually and as needed for the following students:
- Preschool
- Kindergarten
- 1st grade
- 2nd grade
- 3rd grade
- Students receiving special education services
- At the request of a teacher or parent/guardian
- New students
